ABSTRACT

Epstein-Barr virus (EBV) positive diffuse large B cell lymphoma (DLBCL) is a rare type of B cell lymphoma associated with chronic EBV infection. The main subtype is activated B cell-like and it′s invasive. The response to combined chemotherapy is worse than that of EBV negative patients, and the prognosis is poor. As the researches on pathogenesis and biological characteristics of EBV positive DLBCL deepen, new therapeutic strategies are emerging, such as antiviral therapy, monoclonal antibody, inhibitors of signaling pathway and immunotherapy including cellular immunotherapy and immune checkpoint inhibitor. These new therapeutic strategies can improve the efficacy and reduce the occurrence of adverse reactions.

ABSTRACT

Objective:To explore clinical results in reconstruction of finger C-shape soft tissue defect with the wrist crease free flap carrying cutaneous nerve. Methods:From June, 2017 to April, 2019, 7 fingers(7 patients) with C-shape defect were treated with the wrist crease free flap carrying cutaneous nerve. The size of defect ranged from 1.0 cm × 2.5 cm-2.2 cm × 4.0 cm; the flap sizes were 1.3 cm × 2.7 cm-2.5 cm × 4.5 cm. Five fingers had unilateral defect of proper palmar digital artery. Two fingers that had bilateral defect of proper palmar digital artery with poor blood circulation were re-established with blood supply by Flow-through flaps. Four fingers had unilateral defect of proper palmar digital nerves, and 3 had bilateral defect of proper palmar digital artery. Five of the fingers were repaired by the superficial branch of the radial nerve and 2 repaired by palmar cutaneous branch of median nerve. Regular outpatient follow-up was conducted after surgery for 8 to 15 (mean 11) months. Results:All the fingers and flaps survived with primary healing. Numbness existed in the areas of functional dominance of the cutaneous nerve. At the end of follow-up, the flaps showed good texture without significant bloated appearance with the recovery of protective sensation. The sensation of fingertip recovered to S 4 in 5 fingers and S 3+ in 2 fingers. Finger pulps were plump. All of the fingers moved freely. Linear scars were observed at donor sites and the wrists moved freely. Numbness feeling in the areas of cutaneous nerve disappeared at 6 to 8 weeks after surgery. According to the Functional Evaluation Criteria of the Finger Replantation published by the Hand Surgery Society of the Chinese Medical Association, the results were excellent in 6 fingers and good in 1 finger. Conclusion:The wrist crease free flap carrying cutaneous nerve is constant and can be dissected transversely to reconstruct and fit the C-shape defect of finger. It can re-establish the blood supply as well as to repair the proper palmar digital nerve defect at the same time.

ABSTRACT

Gastric cancer is a kind of disease with high incidence and mortality rate, most gastric cancer cases are diagnosed in an advanced, and chemotherapy is the main treatment.Drug resistance is one of the most important causes of therapy failure in gastric cancer patients. It has revealed that the mechanism of tumor resistance mainly involves alterations in cell cycle and proliferation, enhanced DNA repair capacity, defective apoptosis, damaged host immune, altered metabolism of drugs and so on.In recent years,an important molecule-microRNA(miRNA)that regulates the biological behavior of cell has attracted more and more attention.Studies have confirmed that numerous miRNAs can regulate the drug resistance of gastric cancer cells via BCL-2 signaling pathway,PTEN/PI3K/AKT pathway,ABCB1(MDR1/P-gp),autophagy,cell cycle and so on.

ABSTRACT

The present paper is aimedto investigate the effect of basic fibroblast growth factor (bFGF) on proliferation, migration and differentiation of endogenous neural stem cell in rat cerebral cortex with global brain ischemia-reperfusion. A global brain ischemia-reperfusion model was established. Immunohistochemistry was used to observe the pathological changes and the expression of BrdU and Nestin in cerebral cortex. RT-PCR was used to measure the NSE mRNA in brain tissue. The results of measurements indicated that in sham operation group, there was no positive cell in cerebral cortex, and the content of NSE mRNA did not change. In the operation group, the expression of BrdU and Nestin increased significantly at the end of the 3rd day, and peaked on the 7th day. NSE mRNA expression did not significantly increase. In bFGF group, compared with sham operation group and model group, the number of BrdU-positive and Nestin-positive cells increased significantly at each time point (P<0. 05), and peaked at the end of the 11th day, and the content of NSE mRNA increased significantly (P<0. 05). This research demonstrated that the proliferation of endogenous neural stem cells in situ could be induced by global cerebral ischemia and reperfu- sion, and could be promoted and extended by bFGF. In additiion, bFGF might promote endogenous neural stem cells differentiated into neurons.
